Candidate Registration Form

Candidate registration forms are to be completed by those who wish to attend a weekend. Your Sponsor most likely pointed you to this website to download and complete the form. Please read the form carefully, and return it to your sponsor.

Sponsor Commitment Form

Sponsors, you know who you are! After your candidate has completed their candidate registration form and obtained the required signature from their pastor, please forward their registration form along with this sponsor commitment form to Via de Cristo at the address provided in the upper left portion of the form.

Team Form

After candidates have attend a weekend, they are welcome to apply to be on a team. Upon completing the form, please mail it to the address provided in the upper left portion of the form.

If you plan on providing Palanca, please attach this form to your palanca package. Please use one section of the form for each package you are providing.

Communication Information Forms

To facilitate collection of accurate data about our membership, we have created a form to update contact information, including the activities each member has participated in and when. Future weekends depend on this type of information and helps us plan ahead using valid information. Please complete and e-mail this form to communications.
