Candidate Forms

Candidate forms are to be completed by those who wish to attend a weekend. Your Sponsor most likely pointed you to this website to download and complete the form. Please read the form carefully, and return it to your sponsor.

Sponsor Forms

Sponsors, you know who you are! After your candidate has completed their registration form and obtained the required signature from their pastor, please forward their registration form along with this sponsor commitment form to Via de Cristo at the address provided on the form.

Team Forms

After candidates have attend a weekend, they are welcome to apply to be on a team. Upon completing the form, please mail it to the address provided on the bottom of the form.

Communication Information Forms

To facilitate collection of accurate data concerning our membership we have supplied a form to update contact information and activities each member has participated in and when. Future weekends depend on such information and we need to plan ahead using valid information. Please send in or email this form to communications.
